Closed Adoption Records Services

Our goal is to reunite adoptees who are searching for their birth parent(s) or a birth parent who is searching for his or her birth child.  Both parties must initiate contact with Catholic Charities and provide written consent for contact prior to a reunion taking place.  Once this occurs, our Post Adoption Specialists notify both the parent and child before providing any identifying information.

Our Post Adoption Specialists also provide non-identifying information and medical history to adoptees upon written request.

To search for Birth Parent(s) please provide:

  • Your adopted name
  • Current address
  • Contact numbers (home/cell phone)
  • Date of birth
  • Adoptive parents full names
  • Consent letter stating your desire for contact
  • Email address, if applicable
  • Copy of driver's license or other ID (used for identification purposes only)

To search for a Birth Child please provide:

  • Your name at the time of birth
  • Current address
  • Contact numbers (home/cell phone)
  • Child's date of birth
  • Given name of child
  • Consent letter stating your desire for contact
  • Email address, if applicable
  • Copy of driver's license or other ID (used for identification purposes only)

To receive all non-identifying information and medical or to update records please provide:

  • Birth Parent name or Adopted name (whichever is applicable)
  • Adoptive parents' full names (if applicable)
  • Your date of birth
  • Current address
  • Contact numbers (home/cell phone)
  • Email address, if applicable
  • Letter stating whether you do or do not desire contact with birth parent(s) or birth child
  • $75.00 check or money order made payable to Catholic Charities of Louisville (for medical history and non-identifying information only)
  • Copy of driver's license or other ID (used for indentification purposes only)
